Extremely annoying American expression.

Where and when did "My bad!" start? Is it someone's catchphrase? Is it a contraction of "am I bad?"?
It is very irritating.

I always thought 'my bad' meant 'oops'.

It's short for "It is my bad" which is in turn short for "It is my bad use of the English language that is leading me to use this phrase, instead of the more traditional 'Whoopsie Daisy!'"

Or really, it's an annoying substitution of the word "mistake" with the word "bad", despite "bad" really being an adjective, and not a noun.

It's actually a contraction of "my bad choice" and it can apply to words or deeds. It started about five years ago, and has been increasing yearly to the extent it's now universal, even making inroads to Australia.

It's part of the living language of course, and belongs in the same category as:

"24-7",

"OH. MY. GOD.",

"are you serious?"

"loo-oo...ZERRRR"
